If you have a pre-existing condition, PKV insurers are likely to charge you a surcharge or decline your application altogether. Common examples include asthma, a high BMI, Hashimoto's disease, drug allergies, diabetes, a history of kidney stones, elevated uric acid, migraine, psoriasis, and past psychological or psychiatric treatment. Most PKV insurers require a health check before accepting you, and the surcharge can be anywhere between 5 and 30% depending on the condition. GKV providers don't impose similar restrictions.

If you have a pre-existing condition and still want PKV cover, we recommend working with a licensed insurance intermediary. They can tell you whether you can be insured in PKV at all, and can request anonymous quotes on your behalf, known in German as a Risikovoranfrage. That matters, because a rejection from one insurer can make it harder to get cover from another. Asking anonymously keeps your options open.
